All requests for help are important to us.

The Donations Committee reviews all the requests it receives. It then makes a decision based on its eligibility criteria and the funds available each year.

An acknowledgement of receipt will be sent to you. Applications are evaluated three times a year by a selection committee that meets in September, December, March and June. The committee will consider all cases received by the last day of the month prior to the committee meeting, e.g. your application must be received by August 31 to be considered at the September meeting. The committee members scrutinize the applications and do all the research necessary to make the selection. Once the committee has studied your application, you will be promptly informed.

FNAP can financially support certain expenses to improve the well-being of your child and the family (physiotherapy, occupational therapy, kinesiologist, specialized equipment, camp, respite). FNAP can also partially finance certain treatments recognized as effective and not covered by an insurance plan.

please send us the requested documents by mail:

◦ Application form completed, dated and signed

◦ Letter of introduction to the child, family and situation, indicating needs

◦ Diagnosis made and signed by a physician or a recognized medical professional signed by the child’s treating physician (e.g. pediatrician, physiatrist, specialized team, social worker, occupational therapist, physiotherapist, kinesiologist).

◦ Most recent Canada Child Tax Benefit

◦ Most recent notices of assessment for both parents

◦ Quote or other documents explaining your needs and associated costs

◦ Child’s photo (Optional: Enclosing it authorizes us to publish it)

Only completed applications will be considered.
